Elite Chiropractic gladly accepts auto accident cases. We will work diligently to help support you through this unfortunate and stressful event. We work cohesively with your insurance company and/or attorney and will need a few things from you before your appointment.
For you to be seen as an Auto Accident or PI case we need to see you within 14 days of the date of the accident.
WE WILL NEED:
- Florida is a no fault state so well will need YOUR car insurance information as well as the other party involved no matter whose fault the accident was.
- Your adjusters contact information as well as any other party’s adjusters contact information
- Your claim number
- Your drivers license
- Health insurance card (if applicable)
